Resources for Health Professions Trainees Coming to VA | Eligibility and Forms

Application forms must be completed by all clinical trainees participating in programs offered at U.S. Department of Veterans Affairs (VA) facilities. Some forms are required of all trainees. Additional forms may be required depending on your clinical training program.

Males born since 1960, between the ages of 18 and 25, MUST register for Selective Service.

All Health Professions Trainees MUST have a U.S. Social Security Number.
If you do not, please stop here and alert your Program Director of Training.

Application Forms

VA Form 10-2850D, Application for Health Professions Trainees, required for ALL HPTs
Do NOT use VA Forms 10-2850, 102850a or 10-2850c.

The forms and mandatory training below are required for all HPTs.
Additional forms may be required depending on your type of appointment. Any additional forms will be available through your local VA facility.

* Department of Defense Trainees who are not paid by VA are exempt from completing Forms SF-61 and OF-306.

** The only VHA Training Programs exempt from Random Drug Testing per policy are: Clinical Pastoral Education, Social Work, Dietetics, Occupational Therapy, Optometry, Audiology, Speech Pathology and non-Clinical or Administrative.

*** OPM's online eQIP uses an online form that is completed by non-physician-resident health professions trainees who are appointed for more than 180 days in aggregate. Your local Human Resources staff will provide you with an access code and a link to eQIP if you are required to complete the online eQIP.

**** OPM's online eQIP uses an online form that is completed by physician-residents who have a greater than one year, continuous appointment and are at the VA continuously for greater than a year. Your local Human Resources staff will provide you with an access code and a link to eQIP if you are required to complete the online eQIP.
